Apollo HomeCare and Apollo AyurVAID Join Forces to Bridge Post-Hospital Care Gap

Two major healthcare providers have partnered to address a critical challenge in patient recovery: the vulnerable period between hospital discharge and full return to home care.

Apollo HomeCare and Apollo AyurVAID have jointly established an integrated Transition Care service, creating a clinically supervised intermediate environment where patients can be properly assessed, stabilised, and prepared for safe discharge to their homes.

The new facilities operate as a bridge between hospital and home, providing patients with continued medical oversight during their recovery phase. Services available at these centres include nursing care, physiotherapy, rehabilitation programmes, nutritional guidance, and targeted Ayurvedic treatments tailored to each patient's stage of recovery. The primary objective remains consistent: ensuring patients achieve adequate stability and strength before returning home.

Medical teams from both organisations work collaboratively on-site, monitoring individual patient progress closely and responding promptly to any clinical developments.

A distinctive aspect of the partnership involves a unified clinical evaluation process. Patients gain access to combined assessments by specialists trained in both modern medicine and Ayurveda. This collaborative assessment framework informs a customised transition care plan that blends contemporary clinical approaches with appropriate Ayurvedic interventions.

Currently, the Transition Care service operates across three major Indian cities—Bengaluru, Chennai, and Delhi—with multiple beds available at each location.

The service extends beyond local residents to accommodate patients travelling from other states and countries for major medical procedures. This monitored recovery phase allows such patients to fully stabilise before undertaking travel, thereby minimising clinical risks associated with premature discharge and long-distance journeys during the healing process.

The collaboration leverages complementary strengths: Apollo HomeCare brings extensive expertise in post-discharge patient management, while Apollo AyurVAID contributes NABH accreditation, precision clinical protocols, and established systems for documentation and outcome measurement. Together, they have constructed a care model emphasising continuity throughout the recovery journey.
